Application/renewal for UFS access card - Non student/staff users. 



THIS APPLICATION IS FOR CARDS THAT WILL BE VALID FOR TWELVE (12) MONTHS. 



* On submission of the following at the UFS Card Division, Main Campus, Thakaneng Bridge, a dual frequency card will be issued that will be valid for the period stipulated on your application form (for a maximum period of twelve (12) months)..

What must be submitted with this application for a card?

  1. Attach copy of positive identification: ID / Driver's Licence / Passport

Please note the maximum combined file size for all the attachments should not exceed 20 MB.

  1. Cards will be activated according to the information provided
  2. Renewal application will be required prior to expiry date upon presentation of the card for renewal
  3. If specific building / facility access is required, it must be specified
